NYS Education Law governs the frequency and content of physical exams required for school. Education law and regulations of the Commissioner of education require physical examinations* of children when they:
§ Enter school for the first time
§ Are in grades 2, 4, 7 and 10
§ Participate in interscholastic sports
§ Need working papers
§ Are referred by the Committee on Special Education
§ Are deemed necessary by the school staff
*These exams are not necessarily interchangeable, but sports physicals are the most complete.
At C-NS, school employed physicians (who are also practicing medicine right in our community) are hired to come to school and provide exams for those who want them, several times during the year. These dates are scheduled in advance, and publicized for a month in school with signs and daily announcements. Before a student can have their exam, they must bring in a health history completed, signed and dated by a parent or guardian. For winter and spring exams are done during the school day, we need to receive this health history in advance. A nurse will call your student to the Health Office and complete their pre-screening and give them an appointment time to see a doctor.
The student’s physical form actually stays in the school they attend, in their cumulative health record at the Health Office. You are welcome to have a copy of it for your records. Just ask at the time of the exam, or later if you find you need it.
Exams done at school are free of charge. However, they are not as complete as your own doctor would perform and no laboratory tests are done. No immunizations (shots) can be given. If the doctor uncovers a medical problem he/she feels needs attention, you will get both verbal and written explanation of this. Follow up by your own doctor may be required. Please return the referral form to your child’s school nurse.
Physicals expiration information. *For a students participating in interscholastic athletes and co-curricular activities physicals are required every year. If the 12 month period for the physical examination expires at the start of during a sport or activity season, participants may conclude that season without being required to have another physical examination until the next season , as long as a health history up date was completed prior to the new season. * For working papers, New York State requires a health examination 12 months prior to issuance of employment certificate.
