TRANSCRIPT REQUEST FAQs
How do I request my transcript? Click here to download and print the request form. Forms should be photographed, saved as a PDF or JPEG and emailed to me at email@example.com for processing.
If you cannot print the form, please write the following information on a sheet of paper:
- Name you used when you went to school here
- Name now if it is different
- Date of birth
- Year you graduated or last attended
- Information on where you would like your transcript sent, including address
- If your request is urgent, please note that on your request.
- YOUR SIGNATURE (not your typed name)
- Contact phone number/email in case we have questions about your request
- If you would like an unofficial copy sent to you, please indicate that on the form.
- Official transcipts must be sent directly to your college or employer OR can be mailed to you in a sealed envelope.
Transcripts must be requested in writing because we need YOUR signature to release your transcript.
APPLICATION TO A SUNY SCHOOL?
Please utilize the SUNY Portal. Once you let me know you are signed up there, I will submit your transcript electronically.
FEE? None at this time.
How long does it take to get my transcript? Our goal is to process requests within two business days of receipt. Please do not anticipate same day service as it is not always possible.
Can transcripts be emailed? If your college, employer or trade school will accept an emailed transcript, we are happy to do so. Please provide the correct email address for us.
Can transcripts be faxed? Official transcripts cannot be faxed, but if your college, employer or trade school will accept a faxed transcript, we are happy to do so. Please include the name of the person to whom the fax should be directed, the company name and the fax number.
Can someone else request my transcript? Once you graduate, your transcript becomes your personal record. NO ONE, including parents or spouse, can pick up your transcript without your written permission or Power of Attorney.
Can I pick my transcript up in person? You must call (315.218.4003) and make an appointment to pick up your transcript.
Can someone else pick up my transcript? If you are having someone else pick up your transcript, you will have to give them your written permission.
Attended North Syracuse High School? You must contact the North Syracuse Junior High School building at 315.218.3622 to request your record.
Attended Cicero High School? Your record is kept at the Cicero-North Syracuse High School building. Please follow the directions above.
TASC ATTACHMENT R/AGE ELIGIBILITY ATTACHMENT FORMS
To enroll in the TASC program, you may need to complete the Age Eligibility Attachment and/or the Attachment R forms.

Both of these forms require that you obtain a Universally Unique Identifier (UUID) by setting up an account with DRC, the company that administers the TASC™, through their self-registration process. You must register for this ID before the forms can be completed by our registrar. Be sure you keep a record of your UUID as we will not have access to it.
Once you have obtained your UUID and completed your portion of the form, please come to Cicero-North Syracuse High School or email the form to our registrar.
We send final transcripts to the college of choice graduating seniors list on their Senior Data Sheet completed in spring of their senior year. If a graduate changes their plans after that sheet is submitted, he or she must contact the Registrar with the new information.
Transcripts are sent out within 5-7 business days after the date of graduation. The majority of transcripts are sent out electronically during the week folowing graduation, but others have to be sent via postal mail which adds time to the process.
If you receive notification from your college that your transcript has not been received, please feel free to contact the Registrar at 315.218.4003 to confirm it was sent and when.
You should obtain transcripts from your counselor. During the summer, the registrar provides them.
NCAA (National College Athletics Association)
We send your transcripts to the NCAA automatically approximately 5-7 business days after graduation.
If You Attended More Than One High School
If you attended more than one high school, the NCAA needs an official transcript from each high school you attended.
11th Graders Moving to 12th Grade
Students may register with the NCAA before they complete 11th grade, but the NCAA will not review your high school transcript until you have completed six (6) semesters of high school, which occurs when you complete 11th grade. They want to see a transcript with final grades for your courses through 11th grade that includes a list of your 12th grade classes. We send these transcripts out in the fall of your senior year.
NAIA (National Association of Intercollegiate Athletics)
Students who register with the NAIA Eligibility Center must list Cicero-North Syracuse High School in their profile. Once you set up your profile, you can submit a request for transcript. We cannot submit your transcript unless you submit your request through the NAIA Portal, not directly through our office.
EXTERNAL DIPLOMA PROGRAM (EDP) GRADUATES
If you obtained your high school diploma through this program, possibly offered through an employer or BOCES, you may have received a diploma from our high school. These records are maintained at the Cicero-North Syracuse High School building and may be requested using the transcript reequest procedure above.
The NYS Education Department TASC Testing Office is the only source for a for a copy of your GED diploma or GED transcript.
