 The school is responsible for the medical screening of the new pupils entering North Syracuse Central School District and entering grades K, 2, 4, 7, 10.  The medical examination requirement may be met in one of two ways:

1.  A "Physical form" can be obtained from the school the student attends; the health care provider may do the examination and complete the form, which is then returned to the school nurse.  A physical examination completed within 12 months prior to school entrance is acceptable.

2.  If a physical is not on file, the school physician will perform a screening examination during the school year.

Dental Screenings
New York State Law (Chapter 281) permits schools to request a dental examination in the following grade school entry, K, 2, 4, 7, &10. Please forward the dental certificate to the school nurse.

Scoliosis Screening
Scoliosis screening must be administered at least once each school year for all students in grades 5 through 9.

Hearing Screening
Hearing screening must be administered to all students within six months of admission to school in grades K, 3, 5, 7 and 10.

Vision Screening
Screening and recording of the following visual tests must be administered to all new entrants to school within six months of admission.  Tests include a minimum of color perception, distance acuity, near vision and hyperopia. 
 In addition, all students will be screened for distance acuity in Kindergarten, 1, 2, 3, 5, 10 and any other time deemed necessary.
